Licensed Social Worker (MSW)
CHP Home Care & Hospice provides comprehensive home health and hospice services throughout Northwest and West Central Ohio. CHP is currently seeking a full-time/part-time Medical Social Worker serving home health & hospice patients and families in Defiance and Fulton counties and at Defiance Area Inpatient Hospice Center. Masters degree preferred, but not required.
This job requires a person who can work independently, while part of a team approach. As part of our dedicated team of caregivers, you will be part of delivering services addressing the physical, emotional and spiritual needs of patients of all ages and their families both in their homes and at our inpatient hospice center. Our outstanding team delivers a complete range of services including home care, hospice, adult day care, and inpatient hospice.

SALARY: Commensurate with education and experience
HOURS: Full-time & Part-time available
LOCATION: Position available for Archbold & Defiance
Duties include:
- Assessing the social and emotional factors affecting patients and families that impede effective medical treatment, and intervening in crisis situations when necessary
- Participating in the development of care plans
- Counseling patients and families in long-range planning and decision-making
- Assisting patients and families in obtaining needed community resources
- Providing social work services to the hospice team
- Assisting with staff in-service programs and community education
- Assessing bereavement needs and provide counseling services in accordance with the plan of care
MINIMUM APPLICANT REQUIREMENTS
- A graduate of an accredited school with at least a bachelor's degree in Social Work (Master's preferred)
- One-year experience working in social work, which may include an acute care setting
- Currently licensed in the state of Ohio
- Physically capable to lift and transfer patients if necessary
- Current driver’s license and dependable insured automobile
- Must have a positive attitude, fostering a cooperative and positive atmosphere

- Ability to network with community agencies and utilize community resources
- Ability to make evaluations and decisions based on measurable or verifiable criteria
- Ability to communicate effectively, both verbally and in writing
- Respecting the patient’s privacy and maintaining confidentiality
- Must maintain license by obtaining necessary contact hours and competency
