Director, Intake, Admissions & Call Center (Advanced Practice Nurse) 



Location: Nashville, TN  

Status: Full Time 

Days: Monday- Friday 8a-5p

Hours: 40/week

SUMMARY

Coordinates daily operations of Intake, Admissions and Call Center processes and has administrative responsibilities for care and coordination of Intake, Admissions and Call Center staff. Assumes responsibility for the coordination and evaluation of patient care during afterhours.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Utilizes the interdisciplinary model to promote cost effective quality care within the hospice environment.
  2. Promotes and models collaborative interdisciplinary practice processes among care team. Ensures coordinated continuity of care and achievement of optimal outcomes; is a team and consensus builder.
  3. Practices in accordance with the mission, philosophy, policies and ethics of Alice Hospice, Inc., and NHPCO guidelines.
  4. Serves as liaison, educator, and resource for hospice in the professional and community arena.
  5. Works collaboratively with the Vice President of Quality and Compliance to ensure smooth transitions from referral to admission to ensure continual compliance.
  6. Aides in achieving agency goals through support, project work and team building.
  7. Provides clinical and administrative supervision to staff including recruiting, selecting, training, and counseling employees, staff assignment and scheduling.
  8. Manages systems, processes, staffing, budget, and outcomes.
  9. Supervises and allocates clinical staff based on agency needs.
  10. Collaborates with staff of other departments as appropriate.
  11. Ensures Alive Admission visit is consistently performed by each Admission nurse.
  12. Collects and analyzes Referral, Intake, Admission and Call Center data to identify trends and opportunities for improvement.
  13. Is responsible for afterhours communication; ensuring Call Center nurses make accurate assessments and patient care decisions.
  14. Oversees Call Center schedules and adjusts to always ensure adequate coverage.
  15. Serves as clinical leader as designated and is in the full rotation of Director on Call
  16. Coordinates admissions to Alive Hospice in all areas of delivery of patient care.
  17. Monitors customer service-related activities to optimize performance.
  18. Under the direction of the Chief Medical Officer (CMO) or designee, diagnoses medical condition, plans and implements interventions, and evaluates patient response.
  19. Assembles and documents complete patient medical database including health history and physical examination
  20. Assesses eligibility for hospice services.
  21. Documents assessments, orders, interventions, results, and discharge planning in the medical record
  22. Exhibits behaviors that reflect commitment to the philosophy and vision of Alive.
  23.  Participates in medical and nursing research projects, including the presentation and publication of findings.
  24. 24. Remains current on research findings and integrates into practice.
  25. Promotes excellence in nursing practice.
  26. Collaborates with Chief Operating Officer or designee to identify the need for and conduct in services for clinical staff.
  27.  Maintains Advanced Practice Nurse certification and demonstrates continued competency in the management of patient care.
  28. Participates in professional organizations and serves on agency committees.
  29. Other duties may also be assigned.


Requirements

  

SUPERVISORY RESPONSIBILITIES 

Is responsible for the overall direction, coordination, and evaluation of Intake, Admissions, and Call Center teams. Directly supervises Intake, Admissions, and Call Center Teams. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; and addressing complaints and resolving problems. 


CONTINUING EDUCATION 

The agency requires the position to complete 15 hours of continuing education per year covering topics that will contribute to improvements in carrying out the above responsibilities. Regulatory agencies may require some disciplines to have additional hours in order to be licensed or certified.

EDUCATION and/or EXPERIENCE

Advanced Practice Nurse with a current Tennessee nursing license or compact state license. Minimum of three years hospice nursing experience preferred.


CERTIFICATES, LICENSES, REGISTRATIONS 

Must have current Tennessee NP license. Current and valid Tennessee driver’s license 

and automobile insurance as required by Tennessee State Law.
